The instructions that are being added at the end of Question Set Text do not make sense for our questions and applicants are not answering questions properly. We have a set that says Terms and Conditions and then four "Questions" that are single answers of yes I accept/agree and The "Answer 1 Time" tag makes applicant thing they only have to answer 1 of the four questions.
